Melatonin Is a Clock Signal, Not a Sedative

The pineal gland releases melatonin when the retina stops signalling daylight, and circulating melatonin tells the rest of the body that biological night has begun. It is a timing cue. It does not act on the sedative pathways that benzodiazepines or antihistamines act on, and it does not knock you out.

This is the source of most disappointment with melatonin. People take it expecting a sleeping pill, feel little, and increase the dose — which does not help, because the limitation is not the amount of signal but what the signal does. What melatonin does well is shift when you feel sleepy, which is why its clearest evidence is in jet lag and delayed sleep phase disorder rather than in general insomnia.

For chronic insomnia, cognitive behavioural therapy for insomnia is the first-line treatment recommended in major clinical guidelines and outperforms medication for long-term outcomes. Melatonin is not the treatment for that condition and is frequently used as though it were.

Why Lower Doses Work at Least as Well

Physiological night-time melatonin concentrations are low. Doses of 0.3 to 0.5 mg produce blood levels roughly in that natural range; 5 or 10 mg produce concentrations many times higher and keep them elevated well past when they would naturally fall.

Research comparing doses has generally found that low physiological doses are at least as effective for reducing sleep onset latency as higher ones, and there is a reasonable argument that sustained high levels are counterproductive — receptors downregulate with continuous exposure, and melatonin still circulating at 7am is a signal that it is still night.

  • Morning grogginess is the most common complaint and it tracks with dose.
  • Vivid dreams and nightmares are reported more at higher doses.
  • Start at 0.5 mg. If nothing happens, the answer is usually timing or the wrong problem, not more milligrams.

Timing Is the Variable That Matters Most

For shifting a delayed body clock — the night owl who cannot fall asleep before 3am — melatonin works through a phase response curve. Taken in the hours before your natural melatonin onset, it advances the clock earlier. Taken in the morning, it does the opposite. The same dose produces opposite effects depending on when it is taken.

For advancing sleep timing, small doses several hours before the target bedtime are what the research generally uses, not a tablet at lights out. For jet lag travelling east, melatonin in the destination's early evening; travelling west, the picture is different and light exposure carries more of the work. Getting the direction wrong makes the problem worse, which is a genuinely underappreciated point.
