How Photobiomodulation Is Supposed to Work — and What Is Actually Established

The proposed mechanism for red and near-infrared light therapy centres on cytochrome c oxidase, a component of the mitochondrial electron transport chain that absorbs light in the red and near-infrared range. The hypothesis is that absorption transiently alters mitochondrial signalling, with downstream effects on cellular energy metabolism and local inflammatory signalling. That mechanism is plausible and has laboratory support.

The distance between a plausible mechanism and a proven outcome is where most panel marketing lives. Published human trials are meaningfully positive for a narrow set of endpoints: several skin outcomes, some localised musculoskeletal pain and recovery measures, and wound healing. They are thin, small, or inconsistent for the broader claims that sell panels — fat loss, sleep architecture, testosterone, and general "cellular health."

Wavelength: why 660 and 850 keep appearing

Most consumer panels combine roughly 660 nm (visible red) and 850 nm (near-infrared). Red is absorbed closer to the surface, which is why skin studies use it. Near-infrared penetrates further into soft tissue, which is why recovery and joint studies use it. A panel offering only one is not defective — it is specialised. A panel advertising six wavelengths is not six times better; it is dividing a fixed diode budget across bands with thinner individual evidence.

Irradiance is the number that gets gamed

Irradiance — power per unit area, usually quoted in mW/cm² — determines how long a session needs to be to deliver a given dose. It also falls off sharply with distance. A manufacturer quoting irradiance without stating the measurement distance has told you nothing, and quoting it at 0 inches (sensor against the diode) is close to meaningless for actual use.

How to read a panel spec honestly: ask for irradiance at a stated distance you would actually use (typically 6–12 inches), confirm which wavelengths and in what ratio, and treat any claim measured at zero distance as a marketing figure rather than a usable one.

Dose: The Part Almost Every Buyer Gets Wrong

Dose in photobiomodulation is irradiance multiplied by time, expressed in joules per square centimetre. Published protocols vary widely, but a large share of skin and musculoskeletal studies land somewhere in the range of a few J/cm² to a few tens of J/cm² per session. That is a wide window, and it is genuinely uncertain territory.

What makes it harder is the biphasic dose response reported across the photobiomodulation literature: more is not linearly better, and past a certain point additional exposure appears to reduce rather than increase the effect. That is the opposite of the intuition most people bring to a light panel, and it argues strongly against the "stand there twice as long" approach.

A practical starting protocol

  • Fix your distance and write it down. Dose is meaningless without it.
  • Start at the shorter end of the manufacturer's recommended time, not the longer end.
  • Treat one region per session rather than rotating through five in ten minutes.
  • Hold everything constant for four weeks before judging anything. Changing distance, time, and target area simultaneously makes the result uninterpretable.

If you want the session to be measurable rather than a ritual, pick one endpoint you can score — joint stiffness on waking, delayed-onset soreness after a known workout, a photographed skin area under fixed lighting — and track it. Anything vaguer than that will be dominated by expectation.

Safety, Eye Protection, and Realistic Expectations

Red light panels are non-ionising and do not produce the DNA damage associated with UV exposure. That is a real safety advantage over tanning-style devices and it is worth stating plainly. It does not mean the devices are risk-free to use carelessly.

High-output panels are bright enough to be uncomfortable and potentially harmful to look into directly, and near-infrared output is largely invisible, so the usual blink reflex does not protect you. Use the goggles that ship with the panel, or keep your eyes closed and turned away. People taking photosensitising medications — some antibiotics, retinoids, and others — should check with a pharmacist first, since the interaction is with light exposure generally.

  • Stated irradiance at a stated distance. If the distance is missing, discount the number entirely.
  • Wavelengths and ratio. 660 nm plus 850 nm covers the two best-studied bands.
  • Treatment area versus your actual goal. A single knee does not justify a full-length panel.
  • Mounting. A panel you have to hold is a panel you will stop using. Door mounts and stands matter more than a 10% irradiance difference.
  • Warranty and returns. These are LED arrays with drivers; individual diodes and drivers are the parts that fail.

If the budget is tight and the goal is skin, buy the mask and skip the panel — that is the application where the evidence is strongest and where fixed geometry makes dosing repeatable. If the goal is recovery after training, a mid-size panel used consistently on one region beats a full-length panel used occasionally.

Does red light therapy actually work, or is it a placebo?
It depends entirely on the endpoint. For the broader claims that sell panels — fat loss, hormone changes, general energy — the human evidence is thin, small, or inconsistent. What wavelength should I look for?
660 nm and 850 nm are the two bands with the most published data. Red at roughly 630–670 nm is absorbed nearer the surface and dominates the skin literature. Near-infrared at roughly 810–850 nm penetrates further and dominates the deeper-tissue and recovery literature. Panels offering many additional wavelengths are dividing a fixed diode budget across bands with less evidence behind them.
How far should I stand from the panel?
Whatever distance the manufacturer used to publish its irradiance figure, because that is the only distance where their dosing guidance means anything. Six to twelve inches is typical for full-body panels. The important part is holding it constant — irradiance falls off roughly with the square of distance, so drifting from six inches to twelve inches can cut your delivered dose to a fraction of what you think you are getting.
Is more time better?
No, and this is the most common mistake. The photobiomodulation literature repeatedly reports a biphasic dose response, where effect increases with dose up to a point and then declines. Doubling session length is not a free upgrade. Start at the low end of the recommended range and hold it constant.
Do I need eye protection?
Yes, or at minimum keep your eyes closed and turned away. High-output panels are extremely bright, and the near-infrared component is invisible, so your blink reflex does not protect you from it. Most reputable panels ship with goggles; use them.
