What Is Citicoline and How Does It Work?
Citicoline — chemically cytidine 5'-diphosphocholine, abbreviated CDP-choline — is a naturally occurring intermediate in the biosynthesis pathway that builds phosphatidylcholine, the single most abundant phospholipid in neuronal cell membranes. It has been used clinically in Europe and Japan for decades (originally as a stroke and traumatic brain injury adjunct) before crossing over into the nootropic supplement market.
What makes citicoline mechanistically distinct from a simple choline source is that when it's absorbed and crosses into circulation, it splits into two separate bioactive components: choline and cytidine. Both cross the blood-brain barrier and are independently useful.
The Choline Pathway
The choline component feeds directly into acetylcholine synthesis — the neurotransmitter most associated with attention, learning, and memory encoding. This is the same general pathway targeted by other choline donors like Alpha-GPC, though the delivery kinetics differ.
The Cytidine/Uridine Pathway
Once past the blood-brain barrier, cytidine is converted to uridine, which combines with choline (recombined intracellularly into CDP-choline via the Kennedy pathway) to synthesize new phosphatidylcholine for membrane construction. This is the mechanism believed to underlie citicoline's use in neurological recovery contexts — it provides raw material for rebuilding and maintaining neuronal membrane integrity, not just neurotransmitter supply.
The Attention Trial: McGlade et al. 2012
McGlade, Locatelli, Hardy, et al. published "Improved Attentional Performance Following Citicoline Administration in Healthy Adult Women" in Food and Nutrition Sciences (2012, Vol. 3, No. 6). The study was a double-blind, randomized, placebo-controlled, three-arm design enrolling 60 healthy women aged 40-60, randomized to 250mg/day citicoline, 500mg/day citicoline, or placebo for 28 days.
Attention was measured using the Continuous Performance Test II (CPT-II), a standard computerized measure of sustained attention that tracks omission errors (missed targets) and commission errors (false-positive responses to non-targets). After 28 days, the 250mg citicoline group made significantly fewer omission errors (p=0.04) and commission errors (p=0.03) compared to the placebo group.
The Memory Trial: Nakazaki et al. 2021
Nakazaki et al. published "Citicoline and Memory Function in Healthy Older Adults: A Randomized, Double-Blind, Placebo-Controlled Clinical Trial" in The Journal of Nutrition (2021). This trial enrolled 100 healthy men and women aged 50-85 with age-associated memory impairment (AAMI), randomizing 49 to citicoline 500mg/day and 51 to placebo for 12 weeks; 99 of 100 participants completed the study.
Citicoline supplementation produced significantly greater improvement in episodic memory (mean improvement 0.15 vs. 0.06 for placebo, p=0.0025) and composite memory scores (mean 3.78 vs. 0.72, p=0.0052) compared to placebo.
