1. How Ashwagandha Modulates the HPA Axis
The hypothalamic-pituitary-adrenal (HPA) axis is the body's central stress-response system. Under acute or chronic stress, the hypothalamus releases corticotropin-releasing hormone (CRH), which triggers ACTH secretion from the pituitary, which in turn drives cortisol production from the adrenal cortex. Chronically elevated cortisol suppresses testosterone synthesis, degrades muscle protein, disrupts sleep architecture, and impairs hippocampal neurogenesis.
Ashwagandha (Withania somnifera) is an Ayurvedic herb whose primary bioactives — withanolides, withaferin A, and sitoindosides — appear to act at multiple nodes in this cascade rather than at a single receptor. This multi-target profile is what earns it the classification of "adaptogen": a compound that buffers against diverse stressors without overstimulating any single pathway.
The three primary mechanisms identified in preclinical and human studies:
- GABA-A receptor potentiation. Withanolides bind GABAergic sites in the amygdala and hypothalamus, blunting the initial CRH surge in response to psychosocial stress. Bhattacharya 2000
- Reduction of pro-inflammatory signalling (NF-κB, IL-6, TNF-α). Chronic inflammation is both a driver and consequence of HPA axis dysregulation. Withaferin A directly inhibits IκB kinase, reducing NF-κB transcription activity. Kaileh 2007
- Cortisol negative-feedback sensitisation. Ashwagandha appears to upregulate glucocorticoid receptor sensitivity, allowing normal negative-feedback loops to terminate the cortisol response more efficiently. Chandrasekhar 2012
The net effect in human trials is a blunted cortisol awakening response and lower 24-hour urinary cortisol — without the sedative side-effects of pharmaceutical anxiolytics or the adrenal suppression risk of exogenous corticosteroids.
2. Clinical Evidence for Cortisol Reduction
The most-cited double-blind RCT on cortisol is Chandrasekhar et al. (2012), published in the Indian Journal of Psychological Medicine. In 64 adults with chronic stress, 300 mg KSM-66 twice daily (600 mg/day) for 60 days produced a 27.9% reduction in serum cortisol compared to placebo, alongside significant improvements on the Perceived Stress Scale (PSS) and General Health Questionnaire (GHQ-28). A second well-controlled study by Pratte et al. (2014) using 240 mg Sensoril twice daily for 60 days found a 24.2% cortisol reduction and a 44% reduction in serum C-reactive protein, suggesting simultaneous HPA and inflammatory axis modulation. Pratte 2014
A 2019 dose-ranging trial (Langade et al.) compared 240 mg and 480 mg Sensoril in non-restorative sleep patients. Both doses reduced cortisol significantly; the higher dose produced more robust effects on sleep quality (PSQI score −72%) and morning cortisol. Langade 2019
Across the literature, the consistent finding is that cortisol reductions require 4–8 weeks of continuous dosing to manifest. Single-dose studies do not reliably show cortisol effects, which rules out acute stimulant-style mechanisms and supports the receptor-sensitisation model described above.
3. Testosterone, Strength, and Body Composition in Men
Elevated chronic cortisol directly suppresses testicular testosterone synthesis via two mechanisms: (1) cortisol occupies androgen receptors due to structural similarity with testosterone, and (2) glucocorticoids inhibit LH pulsatility at the pituitary level. Any intervention that reliably lowers chronic cortisol should, in theory, create permissive conditions for testosterone recovery. Ashwagandha appears to do exactly this — and possibly more.
Wankhede et al. (2015) randomised 57 young men engaged in resistance training to 300 mg KSM-66 twice daily or placebo for 8 weeks. The ashwagandha group showed: Wankhede 2015

- +18.2 kg bench press strength (vs. +9.3 kg placebo)
- +20.5 kg leg extension strength (vs. +8.7 kg placebo)
- Significantly greater reduction in exercise-induced muscle damage markers (serum creatine kinase)
The mechanism for the testosterone effect is likely multi-factorial: cortisol lowering, direct withanolide interaction with the HPG axis, and antioxidant protection of Leydig cells (which are particularly vulnerable to oxidative stress).
Lopresti et al. (2019) enrolled 43 overweight men aged 40–70 and found that 600 mg KSM-66 daily for 8 weeks increased testosterone by 14.7%, DHEA-S by 18%, and significantly improved fatigue, vigour, and libido scores. Notably, the testosterone effect was accompanied by reduced cortisol and inflammatory markers, consistent with the HPA-mediated pathway. Lopresti 2019
4. KSM-66 vs Sensoril: Extract Comparison
Not all ashwagandha supplements are equivalent. The two most clinically studied proprietary extracts differ in sourcing, standardisation, and human trial profiles:
KSM-66 (Ixoreal Biomed) is a full-spectrum root-only extract using a green-chemistry extraction process. It is standardised to ≥5% withanolides by HPLC and maintains the native ratio of withanolides present in the whole root. The majority of positive testosterone and strength trials use KSM-66 at 300–600 mg/day.
Sensoril (Natreon Inc.) uses both root and leaf material and is standardised to ≥10% withanolides and ≥32% oligosaccharides. The higher withanolide concentration allows effective dosing at 125–250 mg/day. Sensoril has a stronger evidence base for anxiolytic effects and sleep improvement, partly because the leaf fraction contains higher concentrations of withaferin A — the compound most associated with GABA-A modulation.

Look for supplements verified at ≥5% withanolides with third-party testing. 600 mg KSM-66 is the clinical benchmark dose.
5. Thyroid Effects, Anxiolytic Mechanism, and Safety Profile
Thyroid Hormone Modulation
A 2018 pilot RCT (Sharma et al., Journal of Alternative and Complementary Medicine) enrolled 50 patients with subclinical hypothyroidism (elevated TSH, low-normal T3/T4) and found that 600 mg ashwagandha root extract daily for 8 weeks produced significant increases in both serum T3 (+41.5%) and T4 (+19.6%), with TSH declining toward normal range. The proposed mechanism involves ashwagandha's antioxidant effect on thyroid follicular cells and possible upregulation of deiodinase enzymes that convert T4 to active T3. This is clinically relevant for two reasons: (1) people with subclinical hypothyroidism may see compounding metabolic and body composition benefits, and (2) people with hyperthyroidism or Graves' disease should exercise caution and consult a physician, as further T3/T4 elevation could be contraindicated.
Anxiolytic Effects vs SSRIs
Ashwagandha is not a pharmaceutical replacement for SSRIs in diagnosed anxiety disorders. However, the mechanism comparison is instructive: SSRIs block serotonin reuptake, requiring 2–6 weeks for clinical effect and carrying significant side-effect burdens (sexual dysfunction, weight gain, discontinuation syndrome). Ashwagandha's anxiolytic action operates through GABA-A potentiation and cortisol modulation — a fundamentally different pathway.
Andrade et al. (2000) found that Withania somnifera extract produced anxiolytic effects comparable to low-dose lorazepam in animal models, without the sedative or motor impairment side effects. In human trials, Chandrasekhar (2012) showed HAM-A scores improved by a magnitude considered "clinically significant" (>50% reduction from baseline) in the high-dose group, comparable to outcomes seen in SSRI trials for generalised anxiety — but with far fewer adverse events reported. The key practical distinction: ashwagandha works best for stress-driven anxiety in otherwise healthy adults. For clinical anxiety disorders, panic disorder, or PTSD, pharmaceutical intervention remains the evidence-based standard of care.
Safety and Tolerability
Across trials up to 600 mg/day for 12 weeks, adverse events are rare and mild (GI discomfort in ~5% of participants, most resolving within 1–2 weeks). Rare case reports of hepatotoxicity have appeared in the literature; these appear associated with very high doses or adulterated products. No serious adverse events have been reported in any of the KSM-66 or Sensoril RCTs when used at studied doses. Ashwagandha should be avoided during pregnancy (uterine contractile activity in some studies) and used cautiously alongside sedative medications or thyroid-active drugs.
Ashwagandha Cortisol + Testosterone Protocol
- Foundation: KSM-66 ashwagandha extract, 300 mg with breakfast + 300 mg with dinner (600 mg/day total). Take with food to improve absorption and reduce GI sensitivity.
- Duration: Minimum 8 weeks continuous before assessing full effect. Cortisol markers typically shift at 4 weeks; testosterone effects appear at 6–8 weeks.
- Sleep-prioritised variant: Sensoril 250 mg taken 1 hour before bed. Particularly effective if primary complaint is non-restorative sleep and morning cortisol spike.
- Stacking note: Pairs well with magnesium glycinate (400 mg at night) for compounded HPA axis support. Avoid combining with prescription sedatives without medical supervision.
- Cycling: Optional but not required. If cycling, 12 weeks on / 4 weeks off is a conservative framework. Tolerance does not appear to develop in available trial data.
- Biomarker tracking: If using for testosterone optimisation, baseline serum testosterone + cortisol before starting, retest at 8 weeks.
Prioritise supplements listing "KSM-66" or "Sensoril" explicitly on the label with withanolide percentage. Generic ashwagandha powder without standardisation is a poor substitute.
