What VO2 Max Is and How It Is Really Measured

VO2 max is the maximum rate at which your body can take up and use oxygen during progressively harder exercise, normalised to body mass. Measuring it requires analysing the volume and composition of expired air during a graded exercise test to exhaustion — a laboratory cardiopulmonary exercise test with a mask, a metabolic cart and a treadmill or ergometer.

That is the only method that measures VO2 max. Everything a consumer device reports is an estimate produced by a model. The most common approach uses the relationship between heart rate and running pace during submaximal steady effort: if your heart rate at a given pace is lower, the model infers higher fitness. It is a reasonable inference and it is not a measurement.

Why the number is worth caring about anyway

What Your Watch Is Actually Doing

A watch estimate depends on three inputs: heart rate accuracy, pace or power accuracy, and how well you fit the population the model was built on. All three are sources of error, and the first is the largest.

  • Wrist optical heart rate is least accurate exactly when it matters — during rapid changes in intensity and at high heart rates. A chest strap improves the VO2 estimate more than a more expensive watch does.
  • Estimates need steady outdoor running with reliable GPS. Trail running, treadmill work, hills, wind and heat all degrade the model, sometimes badly.
  • Maximum heart rate assumptions matter. Many watches start from an age-based formula whose individual error is large, and if your true maximum differs meaningfully the estimate shifts with it.
  • Between-brand comparisons are meaningless. Different manufacturers use different proprietary models and produce different numbers from the same run.

Read your watch's number as an index, not as a physiological measurement. A rise from 44 to 48 over three months of consistent training is meaningful. The difference between your 48 and someone else's 51 on a different platform is not.

Free Tests That Track Fitness As Well As a Watch

Two classic field tests remain genuinely useful, cost nothing, and are directly comparable to your own previous results, which is the comparison that matters.

  • Cooper 12-minute test: cover as much distance as possible in 12 minutes on a flat track. Distance maps to an estimated VO2 max via a standard formula.
  • 1.5-mile timed run: the same idea from the other direction, and often more tolerable for people who dislike open-ended efforts.
  • Repeated submaximal effort: run a fixed route at a fixed heart rate and record the time. Improvement shows up as the same heart rate producing a faster time — the same logic the watch uses, measured directly.

Standardise the conditions the same way you would for any measurement: same route or track, similar temperature, similar time of day, similar sleep and fuelling, and not the day after a hard session. Heat in particular raises heart rate at any given pace enough to produce an apparent fitness drop that is entirely thermal.

Maximal exercise testing carries real cardiovascular risk for some people. Anyone with known heart disease, uncontrolled hypertension, a history of chest pain or fainting during exertion, or who is sedentary and over 40 should get clinical clearance before performing an all-out test. Improving the Number, and When to Stop Caring About It

The training that raises cardiorespiratory fitness is not mysterious: a large base of easy aerobic work plus a smaller amount of high-intensity interval training. Both matter, and the common error at both extremes is doing everything at a moderate intensity that is too hard to accumulate volume and too easy to drive adaptation.

Improvement is also non-linear. Untrained people can see substantial gains within a few months; well-trained people fight for small increments, and the number is partly genetically constrained. That is a reason to focus on the trend and on training consistency rather than on hitting a target figure.

A useful stopping point: once you are training consistently and the estimate has stabilised in a good range, the number has told you what it can. Continuing to chase a decimal from a modelled estimate is optimising the model rather than the fitness, and the honest measure of progress at that stage is what you can do — the pace you hold, the hill you climb, the session you recover from — not what the watch infers.
