Dr. Rajagopal Sekhar is a professor of medicine at Baylor College of Medicine specializing in nutrition, metabolism, and aging. What sets him apart from most longevity researchers is the rigor and scope of his GlyNAC work: not just one biomarker improved — ten aging hallmarks reversed simultaneously in a single randomized controlled trial.

The Sekhar lab's interest in GlyNAC began not with aging but with HIV. People living with HIV experience accelerated biological aging and have profoundly depleted glutathione (GSH) levels. Supplementing glycine and N-acetylcysteine — the two direct precursors to GSH — produced striking health improvements in HIV patients. Sekhar then asked whether the same intervention might help healthy older adults, who also show dramatic GSH depletion.

The results from healthy older adults were, in Sekhar's own words, even more dramatic. The initial pilot (Sekhar 2021, Nutrients) showed large effects. The follow-up randomized controlled trial (Sekhar 2022, Journal of Nutrition) confirmed them in a properly controlled setting. The scientific community took notice — not because GlyNAC is new, but because no single supplement intervention had previously corrected so many aging hallmarks at once.

What GlyNAC Actually Is

GlyNAC is not a novel compound or patented molecule. It is simply the combination of two widely available, inexpensive amino acid supplements taken together:

  • Glycine (Gly): The simplest amino acid. Conditionally essential — the body produces it but not enough for optimal aging. Major component of collagen (one-third of all collagen amino acids are glycine). Also found in gelatin and bone broth. Sold as a cheap powder.
  • N-Acetylcysteine (NAC): An acetylated form of the amino acid cysteine. NAC has been used clinically for decades — as the antidote for acetaminophen (Tylenol) overdose, as a mucolytic agent in COPD, and as a psychiatric adjunct. It raises cellular cysteine levels.

The reason these two compounds are combined in a specific 2:1 ratio (glycine:NAC) relates to how the body synthesizes glutathione. GSH is a tripeptide — a molecule made of three amino acids: glutamate, cysteine, and glycine. The body usually has plenty of glutamate but is limited by two substrates:

By supplementing both cysteine (via NAC) and glycine simultaneously, GlyNAC removes both rate-limiting constraints on glutathione synthesis. This is why neither supplement alone produces the same dramatic effect — you need both to fully open the GSH production bottleneck.

The Glutathione Deficit in Aging

Glutathione is often called the body's "master antioxidant" — and the label is deserved. GSH is present in virtually every cell, acts as a direct antioxidant (neutralizing reactive oxygen species), regenerates other antioxidants (vitamins C and E), detoxifies heavy metals and xenobiotics, and regulates immune function, protein synthesis, and gene expression.

The problem: plasma GSH declines approximately 30–40% with aging. This is not a minor fluctuation — it represents a profound erosion of the body's primary oxidative defense system. Multiple studies have established plasma GSH as a longevity biomarker; people who maintain higher GSH levels into old age have better survival outcomes.

Why does GSH fall with age? The primary driver is reduced synthesis — specifically, reduced availability of cysteine and glycine, combined with some decline in glutathione synthase activity. The result is a progressive accumulation of oxidative damage that:

  • 1.Damages mitochondrial DNA and proteins, impairing energy production
  • 2.Triggers chronic low-grade inflammation (inflammaging)
  • 3.Accelerates cellular senescence (zombie cells that drive tissue dysfunction)
  • 4.Disrupts insulin signaling, driving metabolic syndrome
  • 5.Impairs muscle stem cell function, accelerating sarcopenia

This oxidative-stress cascade is not a background feature of aging — it is, by the Sekhar lab's hypothesis, a proximate cause of multiple aging hallmarks. Fix the GSH deficit, and you may interrupt many downstream pathways simultaneously.

What Sekhar 2022 Actually Showed

The 2022 Journal of Nutrition paper (Sekhar RV et al., "GlyNAC Supplementation Improves Glutathione Deficiency, Oxidative Stress, Mitochondrial Dysfunction, Inflammation, Physical Function, and Aging Hallmarks in Older Adults") was a randomized, double-blind, placebo-controlled trial enrolling older adults aged 71–80 years across 24 weeks of supplementation. Participants received either GlyNAC (glycine 9g + NAC 6g daily) or placebo.

The breadth of effects matters. This was not a study that found one biomarker improving with marginal statistical significance. Ten distinct aging hallmarks — spanning metabolic, inflammatory, mitochondrial, structural, and functional domains — all moved in the right direction simultaneously. And GSH levels normalized entirely to those seen in young adults, suggesting the intervention was not producing a partial effect but fully correcting the glutathione deficit.

The 2021 pilot study (Nutrients) found similar patterns in a smaller cohort and served as the foundation for the larger RCT. The consistency across both studies is a marker of a real, robust signal.

The HIV Connection: Where GlyNAC Research Began

HIV-positive individuals present a natural model of accelerated biological aging. They experience earlier-than-expected onset of cardiovascular disease, cognitive decline, metabolic syndrome, and frailty — often decades before these conditions would appear in HIV-negative individuals. Alongside these aging-related complications, GSH levels in HIV patients are profoundly depleted.

Sekhar's early work demonstrated that GlyNAC supplementation in HIV patients produced dramatic improvements in GSH levels, oxidative stress, mitochondrial function, and several aging-related biomarkers. The effect was reproducible. This established the proof of concept: correcting GSH synthesis via precursor supplementation could reverse aging-related pathology in humans.

The critical question was whether these benefits would translate to healthy older adults who do not have HIV. The answer, from the 2022 RCT, was definitively yes — and in some respects the results in healthy older adults were even more striking, likely because their metabolic systems were not simultaneously compromised by active infection and antiretroviral therapy.

Why Not Just Take Glutathione Directly?

This is the first question most people ask, and it is the right question. Oral glutathione supplements are widely sold in health food stores. The problem is absorption. GSH is a tripeptide that is largely broken down by digestive enzymes in the gastrointestinal tract before it reaches the bloodstream. You absorb the component amino acids — glutamate, cysteine, and glycine — not the intact GSH molecule.

What About Liposomal Glutathione?

Liposomal delivery encapsulates GSH in a lipid bubble, which can improve absorption by protecting it from GI degradation and enabling direct uptake through intestinal cell membranes. Research shows liposomal GSH does raise plasma GSH to some degree. However:

  • Effects are smaller and less consistent than GlyNAC in head-to-head comparisons
  • High-quality liposomal GSH costs $3–10 per day — up to 20x more expensive than GlyNAC
  • Product quality varies enormously; encapsulation efficiency is rarely verified
  • No RCT with comparable scope to the Sekhar GlyNAC trials exists for liposomal GSH

The precursor approach is more elegant and more cost-effective: supply the body with the specific building blocks it cannot get enough of in aging, and let cellular GSH synthesis machinery do the rest — inside the cell, where GSH is needed.

Each Compound's Independent Benefits

Glycine: More Than a GSH Precursor

Glycine is sometimes dismissed as "just the simplest amino acid" — an undeserved reputation. Evidence for glycine's independent benefits in aging continues to accumulate:

  • Collagen synthesis: Glycine constitutes ~33% of all amino acids in collagen — the structural protein in skin, joints, tendons, and bone matrix. As collagen production declines with aging, dietary glycine deficiency may be a contributing factor.
  • Sleep quality: Multiple RCTs (Yamadera 2007, Bannai 2012) show glycine 3g before bed improves subjective sleep quality, reduces fatigue, and may improve sleep architecture in middle-aged and older adults.
  • Anti-inflammatory: Glycine inhibits NF-κB signaling and may reduce inflammatory cytokine production via glycine-gated chloride channels on macrophages.
  • Metabolic: Glycine reduces serine biosynthesis, which has implications for one-carbon metabolism and potentially cancer metabolism (reduced serine availability may limit rapidly dividing cells).
  • Lifespan extension in animal models: Dietary glycine supplementation extended lifespan in mice in several studies, with effects comparable to caloric restriction in some models.

NAC: A Clinically Established Antioxidant

N-acetylcysteine has a longer clinical history than almost any supplement on the market. As the only approved antidote for acetaminophen overdose, its safety profile is thoroughly documented across decades of clinical use. Its benefits extend far beyond emergency medicine:

  • Raises intracellular cysteine: Primary mechanism; dramatically increases availability of the rate-limiting GSH precursor
  • Reduces homocysteine: Elevated homocysteine is a cardiovascular risk factor; NAC has homocysteine-lowering effects via multiple pathways
  • Neuroinflammation: NAC crosses the blood-brain barrier and reduces neuroinflammation; investigated in depression, OCD, Alzheimer's disease, and schizophrenia research
  • Respiratory: Mucolytic effects are established; chronic NAC use reduces COPD exacerbations
  • COVID-19 research: Multiple papers explored NAC for reducing oxidative stress and cytokine storm associated with SARS-CoV-2 infection

Safety Profile and Considerations

Glycine Safety

Glycine has one of the cleanest safety profiles of any amino acid supplement. No known toxicity at supplemental doses has been documented in human studies. It is classified as GRAS (Generally Recognized As Safe) by the FDA. At very high doses (>60g/day in experimental contexts), no adverse events were recorded. At the 3–9g/day range used in longevity supplementation, it is essentially inert from a toxicity standpoint.

NAC Safety

NAC is extremely well-studied in humans. At standard doses (600–1,800mg/day), side effects are uncommon and typically mild: occasional GI discomfort (nausea, bloating) particularly on an empty stomach. Taking NAC with food mitigates this. A sulfur-containing "rotten egg" odor is sometimes noticed from breath or urine — normal and harmless.

In 2021, the FDA issued warning letters to companies marketing NAC supplements, citing ambiguity about whether NAC could be legally sold as a dietary supplement (since it was investigated as a drug in the 1960s). This generated significant media attention. However, the FDA subsequently signaled it would exercise enforcement discretion and allow NAC supplements to remain on the market while developing new guidance. NAC supplements from established brands (NOW, Jarrow, Thorne, Life Extension) remain widely available and legal to purchase.
