Ashwagandha (Withania somnifera) is an adaptogen — a class of plants that are proposed to help the body resist physiological and psychological stress. In Ayurvedic medicine, ashwagandha has been used for over 3,000 years as a rasayana (rejuvenating tonic). The modern clinical evidence for ashwagandha is more substantial than for most adaptogens: multiple reasonably well-designed RCTs show consistent effects on perceived stress, cortisol, sleep quality, and (in men with suboptimal testosterone) testosterone levels. The evidence quality is moderate — most trials are small, industry-funded, and use proprietary extracts — but the effect sizes are clinically meaningful and the safety profile is excellent at standard doses.
The most important thing to understand before buying ashwagandha: extract standardization matters enormously. Raw ashwagandha powder contains 0.1–0.5% withanolides; standardized extracts contain 2.5–10%. The Chandrasekhar et al. 2012 trial remains the most cited evidence for ashwagandha's cortisol and stress effects. Participants: adults with a history of chronic stress (PSS score ≥20 at baseline). Intervention: KSM-66 300mg twice daily (600mg/day) vs matched placebo. Duration: 60 days. Results: cortisol reduced 27.9% in treatment group vs 7.9% in placebo (p<0.0001). PSS (Perceived Stress Scale) score reduced 44.0% vs 5.5% (p<0.001). The General Health Questionnaire (GHQ-28) improved significantly across all subscales: somatic, anxiety, social dysfunction, and severe depression subscores. Serum CRP (inflammatory marker) also decreased significantly.
Study limitations: small N (64), single-center, industry-linked (Ixoreal Biomed manufactures KSM-66). The cortisol reduction magnitude is large enough to be clinically meaningful even accounting for placebo deflation. The 60-day timeframe is appropriate for assessing HPA axis adaptation. Comparable effects have been replicated in subsequent trials (Pratte 2014, Auddy 2008 Sensoril trial, multiple smaller studies), increasing confidence in the cortisol and stress endpoints.
Ashwagandha can raise T3 and T4 thyroid hormones. Multiple case reports and a 2019 RCT (Sharma et al., N=50) show ashwagandha supplementation increases both T3 (triiodothyronine) and T4 (thyroxine) — potentially beneficial for hypothyroidism, but dangerous for people with hyperthyroidism (Graves' disease, toxic nodular goiter) or those taking levothyroxine or other thyroid medications.

Cycling: The evidence does not support mandatory cycling — most RCTs run 60–90 days continuously. A common practice is 8–12 weeks on, 4 weeks off, though there is no clinical evidence this is necessary. Tolerance has not been observed in trial data. Extended use (6–12+ months) appears safe based on available data.
What to stack it with: Ashwagandha pairs well with magnesium glycinate (for combined sleep and cortisol normalization), phosphatidylserine (further HPA axis modulation — some evidence for further cortisol reduction when combined), and omega-3 EPA (anti-inflammatory and neuroinflammation reduction).
