Ashwagandha (Withania somnifera) is an Ayurvedic adaptogen used for over 3,000 years. Modern clinical research has validated several of its traditional uses — particularly stress reduction, cortisol modulation, and sleep improvement — through double-blind, placebo-controlled RCTs. Unlike many herbal supplements, ashwagandha has accumulated a meaningful body of human trial evidence, not just mechanistic or animal data.
However, not all ashwagandha is equivalent. KSM-66 (Ixoreal Biomed) and Sensoril (Natreon) are the two primary branded extracts with extensive human trial data. Generic "ashwagandha root powder" has inconsistent withanolide content and is not equivalent to these standardized extracts in clinical studies. When evaluating ashwagandha evidence, the extract type matters as much as the dose.
Design: 300mg KSM-66 twice daily (600mg total) vs. placebo for 60 days. Primary outcomes: serum cortisol, PSS (Perceived Stress Scale), safety biomarkers. N=64 adults with self-reported chronic stress.

This trial is the most cited piece of ashwagandha evidence and the data it contains is robust for an herbal supplement.
Design: 300mg KSM-66 twice daily vs. placebo × 8 weeks in adults with insomnia (PSQI score ≥5). Primary outcome: PSQI (Pittsburgh Sleep Quality Index), sleep efficiency, sleep onset latency.
Results: Significant improvements in sleep quality (PSQI total score −72% vs. −29% placebo), sleep efficiency (+6.2% vs. +1.5%), sleep onset latency, and wake-after-sleep-onset. Mental alertness on waking also improved significantly in the ashwagandha group. The sleep effect appears to work via the triethylene glycol component (not withanolides), which has GABA-A modulatory effects.
Data: A 2015 RCT (N=57 male adults, 300mg KSM-66 twice daily × 8 weeks) found significant increases in testosterone (+15% vs. 2.5% placebo, p=0.04), muscle strength (bench press and leg extension), and muscle recovery (muscle damage markers). A separate 2015 RCT found significant improvement in VO2max and cardiorespiratory endurance. Effect sizes are moderate — meaningful for supplementation but not dramatic.
Important context: Testosterone effects are more consistent in men with low-normal baseline testosterone and in conjunction with resistance training. Effects in men with normal-high testosterone are smaller. Effects in women are less studied but HPA normalization (cortisol reduction) may indirectly improve DHEA and estrogen balance.

Primary: HPA axis normalization. The hypothalamic-pituitary-adrenal (HPA) axis governs the stress response — stress signals trigger CRH → ACTH → cortisol cascade. Withanolides appear to reduce HPA axis hyperreactivity rather than simply blocking cortisol production. This "normalizing" effect is the hallmark of adaptogen activity — reducing cortisol when elevated without suppressing it when appropriately needed. This is distinct from drugs like cortisol blockers or corticosteroids.
Secondary: GABAergic modulation. Triethylene glycol (from the leaf fraction in some extracts) has been shown to induce sleep via GABA-A receptor modulation — similar mechanism to magnesium glycinate and benzodiazepines, but far milder and without dependence risk. This partly explains the sleep quality improvement observed independently of cortisol changes.
Tertiary: NF-κB inhibition + anti-inflammatory. Withanolides inhibit NF-κB signaling, reducing pro-inflammatory cytokine production (IL-6, TNF-alpha, IL-1β). Chronic stress drives inflammation partly through HPA dysregulation; ashwagandha's anti-inflammatory effects may amplify the stress-reduction benefit.
Extract: KSM-66 (first choice) or Sensoril. Do not substitute generic powder. Look for the branded extract name on the label, not just "ashwagandha extract."
Dose: 300–600mg/day. The 600mg/day dose (300mg twice daily) is used in most trials and produces consistent cortisol + stress effects. 300mg once daily is sufficient for sleep support.
Timing: Take with meals to reduce GI discomfort. For sleep support, take the full dose (or the second dose if splitting) 1–2 hours before bed. For stress and cortisol, morning + evening dosing matches trial protocol.
Duration: Allow 6–8 weeks minimum to assess full effects. Cortisol changes and PSS improvements were most pronounced at 60 days in the key trial. Acute effects (same-day stress reduction) are not well-supported by evidence.
Cycling: No formal evidence for cycling requirement. Some practitioners recommend 8–12 weeks on, 2–4 weeks off based on traditional Ayurvedic practice. No clinical data indicates tolerance development.
